Apply for a service dog — at no cost to you.

To be considered for a service dog and to see if The American Service Dog Foundation is a good fit for you, we need to learn more about you. After you complete this application, we will get in touch with you for further details.

Eligibility

  • You are a resident of the United States and at least 18 years old (applications for minors are reviewed with a parent or guardian).
  • You have a documented physical, psychiatric, or neurological disability that a trained service dog could help with.
  • You are able to meet a dog's daily needs — food, exercise, grooming, and routine veterinary care — with support if needed.
  • Your home environment is stable, safe, and welcoming to a working dog.
  • You are able to participate in hands-on team training with your dog before placement.

What happens next

  1. We review your application and reach out with questions.
  2. A conversation about your daily life, needs, and home.
  3. Matching with a dog in training whose task work fits your needs.
  4. Team training together, then placement — free of charge.
